Directions to our school
Our H1N1 Vaccination Clinic has been scheduled for
NOVEMBER 10
A few things to remember:
• If you have already sent in your consent forms, your child will receive the shot at our clinic on the 10th.
•
If you returned consent forms and have ALREADY gotten the vaccine elsewhere, please notify our nurse Margaret Koval at 823-4800 right away so your child does not receive another dose.
• If you did not return consent forms and would like for your child to be vaccinated, you may download a consent form from our school web site and return it to our main office as soon as possible.
