2009-10 SAFE SCHOOLS/HEALTHY STUDENTS SURVEY

Survey Explanation

The purpose of this survey is to measure progress in achieving the goals of the Safe Schools/Healthy Students Project in Charlottesville and Albemarle schools. These goals include: (1) reducing bullying and building a positive school climate; (2) reducing use of illegal substances such as alcohol, tobacco, and marijuana. This is a four-year project funded by the U.S. Office of Safe and Drug-Free Schools. The project will include schoolwide bullying prevention efforts as well as new counseling and educational programs.

The survey is being administered to all students in grades 3-12, although parents have the right to opt out of the survey. The survey is anonymous, which means that students will not report their names on the survey and there will be no means of identifying individual student answers to the survey. 

 

General Instructions

1. The survey you have received is LIVE.  Once the Submit icon has been clicked, the information will be recorded in the database.

  

2. Warning: Do Not Use the Back Button:  When taking the survey, clicking on the browsers built in "Back-to" icon will cause the survey to end without recording any of the answers. If this happens, you will have to restart the survey from the beginning. If you need to go back to a previous page, use the <<Back button at the bottom of the screen.

 

3. The survey will not record any information until the Submit icon is pressed at the end of the survey.
