Information Concerning the H1N1 Vaccination Clinic

The H1N1 Clinic Scheduled for Monday, November 9th has been postponed. At this time, we do not have a reschedule date for the clinic.

If you have already sent in your consent forms, your child will receive the vaccine at our rescheduled clinic.

If you returned consent forms and have ALREADY gotten the H1N1 vaccine elsewhere, please notify MHS at 244-3100 right away so your child does not receive another dose.

If you did not return consent forms and would like for your child to be vaccinated for 2009 H1N1 influenza, you may download a consent form from the MHS web site and return it to our main office as soon as possible.
